This application is intended for prospective
mentees who wish to participate in the program.
Below, you will find approximately 20 questions (in addition to some brief demographic information) designed to help us understand your aspirations and needs. To ensure we can make the most supportive and effective pairings, we kindly ask that you provide as much detail as possible in your responses to the qualitative questions.
Your thoroughness will allow the OOTA Mentorship Committee to carefully consider your individual goals, capacity and availability, and ultimately help us facilitate a mentorship experience that truly aligns with what you intend to gain from your participation.
