E-Ticket Request

Whether you have a computer or printer problem, would like to have software installed, or need a cartridge, please fill out the form listed below. As soon as we receive it, we will submit your request to the Technology Department. When you write a description of the problem, please provide as much detail as possible. YOU WILL FACILITATE THE SOLUTION BY PROVIDING AS MUCH DETAIL AS POSSIBLE. In the case of printer cartridges, we will fill your request from what is in stock in the AV office. If it is out of stock, we will request it from the Technology Department. Please be patient if a printer cartridge is out of stock.
