By completing this form, I hereby agree that the information supplied is current, I accept the Sun Valley Way and I allow my child to take part in the Sporting Activities provided at Sun Valley Primary School as well as Away Fixtures.
I fully understand and accept that all the activities in which he/she will participate, and undertake, are at my child’s own risk.
I hereby indemnify/hold harmless/absolve the staff, other members of the Sports Team and coaches, assigned parents providing lifts, the Western Cape Education Department, Sun Valley Primary School, including the Principal, Governing Body or Members of Staff against any or all claims that may arise in connection with the loss of, or damage to, the property of, or injury to, my child, in the knowledge that the Sports Department, Coach and School will nevertheless take all responsible precautions for the safety and welfare of my child.
