This is the official NYDKC Fundraising Report Form (FRF) for the 2017-2018 service year. Use this form every time your club plans and hosts a fundraiser. Please note that a formal proof is no longer necessary, but do note that your club advisor and Lieutenant Governor will receive a confirmation email.
