ESYO Summer Instrument Rental Agreement
Thank you for giving your student the opportunity to continue to improve and play! 
Please fill in the following form to rent your ESYO instrument over the summer.

Current students must return their instrument by June 15th. The summer rental period is June 19 - Sept 14, 2024 and the fee is $50.00.

Once the form is submitted, the office will email you an invoice and payment will be due upon receipt. You may pick up your instrument from June 19-21 once payment is made at the ESYO office at 2295 Four Oaks Grange Rd (Bailey Hill elementary - not the main entrance, walk up to Rm #21 for the ESYO office).
