SHFB Partner Agencies - Monthly Participation Report
As a Second Harvest Food Bank Partner Agency, you are REQUIRED to report your numbers served on a monthly basis, per your contract with Second Harvest. Even if the number of clients you serve does not change, you are still required to submit this report every month. This report ensures that we continue to receive funding from cities, the county, and private donors to continue our programs.
This report is due by the 7th of each month (reporting for the previous month).

Number of individuals
Enter the number of UNDUPLICATED clients served in the month (i.e. if a client receives food several times in a month, they are counted only once). Count all family members in a household.
Number of families
A single individual counts as a family.
Number of meals
CONGREGATE FEEDING SITES (SOUP KITCHENS) ONLY. Report all meals that were served.
Do you need to report data for another program?
Never submit passwords through Google Forms.
