FPC After School Tutoring Permission Form
School Sponsored Activity Parental Authorization form
Pursuant to Rule 6A-10.089 (5), F.A.C., Flagler County Public Schools requires parental authorization prior to students participating in school sponsored events and activities. Each student participating in the event or activity listed below must have this form completed and submitted prior to participating. 

Description of the Event or Activity: After School Tutoring for Algebra 1, Geometry, Algebra 2, Upper Level Math, Science, Social Studies, English, and classrooms assessment make-ups/retakes
OR
*This also includes any teacher who is supporting students after school with homework help, additional test retakes or make-ups.

Date(s) and Time(s): Every Tuesday and Thursday from 3:00-4:30 (then students will be walked to the activity bus/car rider pick-up area)
OR
*This also includes any teacher who is supporting students after school with homework help, additional test retakes or make-ups that could occur on days other than Tuesdays and Thursdays.

Algebra 1: ONLINE & On Campus
Geometry: 7-127
Algebra 2: 2-227 (Tuesdays)
Upper Level Math: 2-221 (Tuesdays)
Science: 2-211
Social Studies: 2-223
English: 2-121
Classroom Assessment Make-Ups/Retakes: 1-104
