You understand and agree to abide by this code, and you recognize that failure to do so will result in the outlined consequences. You understand the demands and high expectations that will be required of you should you make the squad. You have discussed these with your parent(s) and feel you can completely fulfill your obligations as a cheer or songleader. Student's name should be typed in below.
By filling out the parent name on this document, you are indicating that you authorize your child to tryout to be a cheerleader/songleader at Yorba Linda High School for the 2022-2023 school year. You have read the Cheer Philosophy and Code, and understand the commitments that will be expected of your child should he/she make the cheer/song squad. You have discussed these things with your child, and we feel we can meet these obligations. You understand there are physical risks involved in the tryout process as well as participation in the program should your child make the squad. You understand that your child must be cleared to tryout including submitting a clearance packet, DMV paperwork, an up-to-date physical, and copy (front and back) of his/her health insurance card, before he/she can participate in tryouts or practices. You believe your child is physically and emotionally able to meet the demands expected of him/her. By putting your name to this form, you are prepared to comply with the Cheer Philosophy and Code, Cheer Contract, and the rules and regulations of the cheerleaders of Yorba Linda High School in the event that your son/daughter should become a member.
