This form is for a client to complete (with or without assistance) to apply for the transitional housing program in Sun Prairie, WI.  There is an additional form required to gain access to the waiting list.  A referral from a 3rd party Social Worker, Case Manager, Etc is needed.

*SFTSM is a faith-based program, however, participating in prayer and any other religious activities is completely voluntary. We serve single parent led households with children under the age of 18, regardless of religious identification or lack thereof.
