Welcome to the new Gluck Evaluation Online Form. We hope this option provides a more convenient method for tracking our Fellows' impact in the community. Fill in requested information below.

The four Student Response questions at the bottom can be based on an in-class discussion you have after the Fellow leaves, or responses you have them write in an exercise. We only need one representative response that best sums up what the students thought about each question.
