YOUTH WEEKEND AUGUST 7 to 9, 2020 in HALIFAX


Those attending the HALIFAX Youth Weekend will be staying 2 nights at SAINT MARY'S UNIVERSITY, 923 Robie Street, Halifax NS Canada B3H 3C3 The weekend will include spiritual discussions and a divine service. Activities may include: boat tour, hiking and exploring.

Details of the weekend itinerary for Halifax will be sent after your registration is confirmed.
Departure flight will be during the day of August 7th returning on August 9th

Cost is: $100.00 per person attending in Halifax $275.00 for those flying
$150.00 for new confirmands who are flying

RELEASE
In permitting my child (“my child”) to attend the New Apostolic Church Canada (the "Church") DOTY Youth Weekend in Halifax NS August 7, 8 & 9, 2020 (the “Trip”), I, the undersigned, permit my child to participate in the full range of Church activities. I acknowledge that there are certain inherent risks involved in participating in the Church activities. I release the New Apostolic Church Canada, its employees, volunteers and chaperones attending the trip from liability for any loss, injury or damage to my child, personal property of my child, or death of my child, resulting from participating in the activities on the trip.
INDEMNITY
In consideration of my child being permitted by the Church to participate in its activities and to use its equipment and facilities, I agree to indemnify and hold harmless the Church from any and all claims, causes of action, actions or demands whatsoever which are in any way connected with such use or participation by my child.
AUTHORIZATION
I authorize the Church in the event of an accident or illness affecting my child to authorize all procedures necessary, as the Church may deem essential for the care and well being of my child. If I cannot be reached, I authorize the physician or medical personnel selected by the Church to arrange or provide necessary transportation, to secure and administer treatment, including hospitalization, order x-rays and other routine tests, order injections, anaesthesia or surgery for my child. I understand that I am responsible for any medical fees or prescriptions arising from injury or illness that may occur.
PERMISSION
Photographs or videos in which my child appears may be used by the Church for publicity purposes. With respect to the provisions of the Personal Information Protection and Electronic Documents Act, I give my permission for the Church to collect, use and disclose the personal information contained in this form only, in order to better meet the service needs of myself and my child. Further I give my permission for the Church to maintain personal information concerning me or my child already on file. I understand that I can (a) obtain information concerning the privacy policy of the Church, (b) review and amend my or my child’s personal information records maintained by the Church, and (c) request that my and/or my child’s personal information records be removed from the database of the Church by writing the Privacy Officer at 319 Bridgeport Road, Waterloo, Ontario N2J 2K9.

CODE OF CONDUCT (you must agree to the Code of Conduct to participate in the DOTY Youth Weekend)
The purpose of the DOTY Youth Weekend is that each young person draws near to God, their youth leaders and fellow youth through fellowship, life skills, teambuilding and a divine service especially for the young people.

To accomplish this, the weekend is a retreat, providing a unique environment away from daily life. To preserve this, the following is what is expected of each attendee:


1. I will respect my youth leaders and the volunteers by participating in a positive manner in all activities. I will follow verbal and written directions.

2. I will respect myself by wearing dignified clothing, which is also free from offensive messages.

3. I will respect my body by abstaining from alcohol or any controlled substances. I understand there is a "Zero Tolerance" policy on controlled substances (including alcohol). Should I violate this policy, I will be sent home at my own expense.

4. I will respect others and myself by not using profanity.

5. I will respect the privacy of my fellow youth by only using the room assigned to me. I also know under no circumstances are there to be members of the opposite sex inside assigned rooms.

6. I will respect fellowship by engaging with my peers. I realize the use of cell phones, headsets, music/audio players and other multimedia equipment distracts from this fellowship, and will limit my use of these items. I will respect the divine service and I will not use electronics during that time.

7. I will respect the venue’s rules and the local laws. I know smoking is permitted only in designated areas by those legally permitted to do so.

8. I will respect the weekend by not leaving the property or the group.


By my acknowledgment and respect of the above, I understand that there will be consequences if I violate this code of conduct.
