Subjects will be informed with a terms and agreement page before the survey is taken. The following terms and agreement will be included on the first page of the survey. The purpose of this research project is to conduct a study on clients’ experiences with their mental health professionals and mental health matching applications. This research is conducted by Santa Clara University's Bio-Innovation Design Lab. Your participation in this research study is voluntary. If you decide to participate in this research survey, you may withdraw at any time and your responses will be excluded from the study. The procedure involves filling an online survey that will take approximately 2 minutes. We will request your email if you agree to an optional follow-up interview at the end of the survey. Your responses will be confidential and we do not collect identifying information such as your name. The survey will consist of brief background questions, experiences with your mental health professional, and the use of mental health matching platforms. Your answers will be stored in a password-protected electronic format. To help protect your privacy, the surveys will not contain information that will personally identify you.
