Summer School 2018 - Request for transportation to/from alternate address
Please fill out this form completely. You will need to fill out a separate form for each student. Requests will only be approved if the alternate address does not change the student's summer school placement and if the alternate address is in the eligible busing area for your student's summer school. You will be notified via email no later than May 18th, 2018 of approval or denial. Busing information will be sent with your student's confirmation letter in June for those that are approved.

Requests must be submitted no later than May 4th, 2018. No new requests will be accepted after this date.
