The Zimbabwe Cultural Society of BC (ZICUSO BC) in collaboration with Adler University (Vancouver) are conducting research on how social determinants of health (e.g. employment status, housing, immigration status, ethnicity, culture, health access and others) affect the black immigrants in British Columbia. As part of this research we request you to answer the questions in this questionnaire.

All answers submitted will be anonymous. This means there will be no personal information attached to the responses you submit as stated in the consent form you have signed.

You can withdraw from this research at any time. It will not be possible to know which answers you have submitted after you click the submit button. Those answers will remain part of the research.

Please answer as many questions as you can. However, you are required to answer all demographic questions in order to proceed to the rest of the questionnaire. As with the rest of the questionnaire the demographic questions are also anonymous and will assist the research team with determining which immigrant groups are mostly affected by the different social determinants of health.
