As a student who is at least 18 years of age, I assume all responsibility and risk while training at West Texas Jiu-Jitsu Academy. If the student training is under the age of 18, I attest that I am the parent or legal guardian and consent to their training.

I understand that Jiu-Jitsu can cause serious injury or even death if proper implementation of instruction is not followed by the student. I agree that sportsmanlike conduct is required at all times during training. In the event of poor conduct by the student, they may be asked to leave class. I understand that there are inherent risks with combat sports, and therefore I agree to listen to all safety instructions and guidelines provided by the instructors. Students may be sparring with other people, but are not required to participate if they do not wish to. I understand that while training at West Texas Jiu-Jitsu Academy, every effort to maintain safe practices will be employed.

Therefore, I do hereby for myself, my heirs, executors, and administrators release and hold harmless West Texas Jiu-Jitsu Academy and its members, the head instructor Roger Del Campo, and any guest instructors from any and all liability, negligence, claims, demands, actions, or causes of actions, or caused of actions resulting from any participation in this sport/activity/class and associated activity.
