Pipeline Incident Report Form
This form has been designed to help the public report their observations of pipeline construction noncompliance and pollution incidents. At this time it will be monitored by the ABRA's Pipeline CSI, Mountain Valley Watch, Trout Unlimited, and West Virginia Rivers Coalition. You may be required to sign in to a Google account to utilize this Pipeline Compliance Surveillance Initiative onlin form. Please direct any questions or concerns you have to pipelineincident@gmail.com or one of the above listed monitoring organizations and we will get back to you as soon as possible.
