The undersigned understands that it is not possible for Saratoga Voices, Inc. to prevent its members or other persons from becoming exposed to, contracting or spreading COVID-19 while participating in or being present at any of its activities, including such activities as rehearsals, meetings and performances. With the above understanding, the undersigned voluntarily assumes the risk of being exposed to, contracting or spreading COVID-19 by participating in the activities of Saratoga Voices, Inc. The undersigned further releases and waives any right to bring suit or claim of any kind or nature whether known or unknown, foreseen or unforeseen, against Saratoga Voices, Inc., its officers, directors, employees, agents or other representatives in connection with exposure and/or spread of COVID-19 related to my participation with or presence at any activities of Saratoga Voices, Inc. *
