Thank you for your interest in TRANSIT ARTS and The Glow Project (an expansion of "Black Girls Glow+). BIPOC girls (ages 11-22) have the opportunity to participate in this special project, as well as all programs of TRANSIT ARTS, a citywide youth arts development program of Central Community House. Please read and complete the following information and provide a parental/guardian signature as acknowledgement and consent for your child to participate within our programs.
The Glow Project is provided at no cost to our participants thanks to support by our generous funders.
These forms are crucial for both emergency contact information/permissions, and to ensure that TRANSIT ARTS workshops stay accessible (free) to the public. Our funders require this information to show that we are making a difference. All information is strictly confidential and reporting is anonymous.This form should be completed by the participant's Parent(s)/Guardian(s).
