JMB Registration Form 2024
Please complete this registration form for each student joining the 2024 Jenison Marching Band. This online form, the three paper forms (listed below), and your $200.00 non-refundable deposit are due Tuesday, April 9, 2024.

In addition to this online form, there are three paper forms that need to be completed and turned in for each student. Please use blue or black ink for all paper forms. These forms were distributed to students during band class in the marching band paperwork packet, and can also be found at jenisonbands.org/downloads. The three paper forms are:

1. Parental Consent Form for Medical Treatment*
2. Acknowledgement and Agreement Form
3. Code of Conduct 

*The Parental Consent Form must be notarized with either an ink stamp or a raised seal stamp. This service is available at area banks or credit unions. Very Important: Do not sign the form until you are in front of the notary, and bring your driver's license. These forms are necessary for medical treatment within Michigan and especially for any out of state trip.
