Bus/Lunch/After Care/School Clothes Application Form (from January 2020)
Please submit this form to apply for all school services from January.

Please note that unless "ANNUAL" service is signed up in September, school WILL NOT extend the service automatically. The service will be CANCELLED unless the form is submitted. Cancellation of the service is available upon request; refund will be made from the following month.

Deadline for sign up is November 17th. Sign up is still available after this date but the service is not guaranteed.

Invoice will be sent in November. Please make payment by the due date which is stated on the invoice.
