I acknowledge that I have read and understood this waiver and that I, the undersigned, on behalf of myself, my heirs and next of kin, voluntarily and knowingly hereby release, waive and discharge Springfield Women’s Hockey Association and all associated parties from any and all liability, claims, demands, losses, damages, costs, expenses, medical attention and actions of any kind, including but not limited to personal injury, death, equipment or property damage, or loss, arising out of or in any way connected with my participation in hockey activities.
