A program representative will need to contact you to arrange picking up the items.  There may also be questions regarding items on the request form.  Would you prefer to be contacted by phone number OR email?  Whether by phone or email, please be looking for a response from a program representative on how to receive your requested items.  If we do not hear from you by our requested date, it may result in your request for items not being fulfilled. *

We will attempt to fill each requested item but we are limited to what we have currently in stock and the number of requests we receive.  If there is a particular need concerning a product (aluminum-free or unscented deodorant, unscented laundry soap etc.) or multiple people in the home using a particular item, please indicate this and the particular product involved at the bottom of the form under "Comments".  We will do our best to fulfill your request.

Comments--This is where you can tell us the number of family members using a particular product or for particular requests.  For instance, if there are three teenagers and an adult using shampoo, we may be able to send more than one product home.  There are also products that companies target to males/females because of scents etc. (deodorant, some shampoos), so if there is a request, please let us know here.  If there is an allergy or sensitivity to a product (deodorant, baby wipes, laundry soap) please indicate this here.  Whatever the particular request, please tell us here.  IF YOU HAVE COMPLETED A FORM FOR A PREVIOUS REQUEST, PLEASE KNOW THAT WE DO NOT KEEP THIS INFORMATION ON FILE--YOU WILL NEED TO GIVE THIS INFORMATION  FOR EACH FORM YOU FILL OUT.  If you do not have any specific need, please indicate n/a.   *
There are many supports available for students/families at Maine South.  If it is deemed that more supports should be given, a Student Support Team contact will be made.  If you have any questions regarding this program, please do not call the school.  The South Promise contact is jroddick-small@maine207.org.   Please give at least one school day response time.
