As purveyors of #SafeStay #SaferBaliwag , we want to provide a safe environment and experience to all our clients, their family and guests as we resume mounting leisure staycation.

In line with this and as part of the OVERNIGHT AND DAY TOUR GUEST, we would appreciate it that ALL COMING GUESTS could please read through this document, accomplish and submit to us within 14 days prior to your visit.

Please answer the Health Check Questions  and make us aware immediately if your situation changes.

NOTICE: The Hacienda Galea Resort and Events Place (HG) is strictly implementing a "NO-FACE-MASK, NO-FACE-SHIELD, NO-ENTRY" policy. Wear your face mask and face shields at all times while inside the HG premises  except when eating or drinking, within their room, or engaging in strenuous physical activities.

Please be advised that the information collected shall only be used in relation to the HG COVID-19 internal protocols in accordance with the Data Privacy Act, and that you are required by RA 11469, Bayanihan to Heal As One Act, to provide truthful information.
