A parent or legal guardian must complete and submit this form as far in advance of your camp dates as possible and no later than 14 days prior to your camp. Before submitting, save a copy of this completed information for your records. This information is (1) for emergency use, and (2) to get to know your camper better, and the directors and counselors will keep sensitive information confidential. Provide any changes to this information at check-in on the first day of camp.

Insurance is not required for camp attendance, and Camp Bethel only provides limited secondary accident insurance. If you have insurance and have plans to be away or unreachable during your child's camp time, you must email a scan or image of the front & back of your family/child medical insurance card to In any emergency, illness or injury, we will immediately contact the parent/guardian.

On Home-in-the-Woods night (Tuesday or Wednesday), when your camper's group sleeps outside under tarp shelters, your child may choose to NOT sleep outside with their group and instead sleep in a male/female cabin with our Leadership Team. Campers who opt out will remain with their group until bedtime when they are picked-up by the Leadership Team and dropped back off to their group at breakfast the following morning. We want EVERY child to feel comfortable and have FUN during their time at camp.
