NEW: Instead of paying at the time of sign-up as you have in the past, please wait until the club has been confirmed. If you do not receive an email by the end of the day the Friday before clubs begin, the club is a go. If a club does NOT reach it's minimum number of participants, you will be notified via email. Once the club is a go, you can make a payment. We prefer the payment be made via NetClassroom. NetClassroom can be accessed through the Parent Portal on the FSA website. Click on Billing to access to make a payment. The statement will not appear right away but you can simply enter the amount owed, comment that payment is for clubs, and it will be applied to your account. If you choose to pay via check, you may put it in the drop box on the wall at the front desk. Please make sure to put your child's name on the check. If you pay via check, please be aware that we will hold the check until we are certain the club has reached the minimum number of participants.
NEW: If you request financial assistance for a club, you must be signing your child up for only ONE club per session. For instance, if you register your child for Lego Robotics and request financial assistance, Lego Robotics must be the only club your child attends during that session. This is to ensure that all children who would not otherwise attend a club have the opportunity to do so with financial assistance. Please let Anna know if you have any questions regarding this new policy.
NEW: Please note the age ranges for all the clubs, some past clubs have updated their age ranges.
NEW: Once the sign-up deadline has passed, the Google sign-up form will no longer be accessible. You must then email firstname.lastname@example.org.
OTHER IMPORTANT INFORMATION:
-Please use one form per child.
-The deadline for Session 4 club registration is FRIDAY, MARCH 17, 2017. We have instructors from off campus who must know the status of their club by the end of the day on Friday prior to the club start date. Please make sure to sign-up by the deadline date of Friday, March 17th to ensure all of our clubs have the opportunity to succeed.
-If the minimum number of students is not met for a club, or the maximum number has already been reached, you will be notified by the end of the day on Friday, March 17th.
-If you are registering your child for a sports related club, you must also sign a sports waiver (if you have not done so already). We require one sports waiver per student per year. You can find the sports waiver in the email with this form or at the front desk.
-Kung Fu and KidznKicks have two options on this form, one option includes the mandatory uniform (t-shirt) and one option does not include the mandatory uniform. Please only choose the option without the t-shirt if your child already has one that he or she can wear.
-If you are registering for private music lessons, you MUST sign-up on this form then contact the instructor immediately (all instructor information is in the club brochure) to set up a time/day for your child. If your child has taken lessons with the instructor in past sessions, you still need to sign-up on this form each session. After filling out this form, you then contact the instructor. The instructor will provide FSA with a final list of students and at that time you will be charged.
