Ontario municipal election rules require the campaign to maintain information about each contributor, confirm that contributions are eligible and issue a receipt for every contribution received. The information collected through this form will be used only to administer your contribution and meet campaign-finance requirements. 

This form does not transfer funds. After submitting it, you will receive instructions for sending your contribution by Interac e-Transfer.

Municipal campaign contributions are not tax-deductible.
