Windham Southeast Supervisory Union              Ridership Collection Form SY20-21

Windham Southeast Supervisory Union is working closely with our school bus provider, F. M. Kuzmeskus, Inc., to ensure a successful transition into school-year 2020-21. Operating within the guidelines provided by our state and federal agencies, we've established safety protocols that begin on the school bus and extend throughout the day.

Gathering details about your student's busing needs will allow us to formulate responsible school bus routes, correlate our bell times with staffing requirements and other resources, remain reactive should we need to enact contact-tracing procedures, and ensure that each student who needs a ride to school will be afforded a seat on a school bus. We understand that some families will elect alternative modes of transportation, but we also understand that this may not be feasible for everyone.

INSTRUCTIONS
1) This collection will be used in conjunction with our contact-tracing procedures. Include each school-aged student in your household, regardless of their transportation needs.

2) Answer each section to the best of your ability. You will be able to update your details by re-submitting your response. Should you need to change your entry after the deadline date, you may do so by contacting F. M. Kuzmeskus, Inc. directly.

3) Space is provided for up to nine (9) students. Should you need to add more students, you will be given the opportunity to 'Submit Another Response' after you finish submitting your first nine (9) students.

4) You may only register one (1) pick-up/drop-off location per household (more details below).

5) You must complete all required (*) sections.
