Medical Waiver and Release of Legal Liability

I recognize that there are certain risks, which are inherent in ice hockey and off-ice training activities.

I further recognize that these risks cannot be eliminated regardless of the care taken to avoid injuries.
On my own behalf, and on behalf of my child and on behalf of my successors, representatives, family, heirs, assigns, and estates, I acknowledge and understand that the decision to participate in the camp places the risk of serious injury, illness and liability. I voluntarily accept and assume all risks arising out of my child’s participation in the camp activities, including, but not limited to property damage, personal injury, and death.

I request that my child (identified above) be permitted to participate in any activity and agree to the following: In consideration for my child’s participation in this activity, on behalf of myself and my child, I waive, release, discharge, indemnify, and hold harmless the camp, coaches, instructors, volunteers, employees and agents from all liability, claims, cost and expenses arising out of these activities which may result in injury or illness to my child.

This waiver is to remain in effect for all camps, morning skates, and any activities hosted by "Coast 2 Coast Player Development", "Coast 2 Coast Hockey Institute", or "Coast 2 Coast Academy" that the participant is enrolled in.

I understand that Coast 2 Coast Hockey Institute and Coast 2 Coast Player Development (the “Hockey School”) And it’s principals and staff are following state and federal public health guidelines and that the Hockey School is going to be offering limited training with specific guidelines.

I am being offered the opportunity to participate in the Hockey School’s limited COVID-19 training program and activities (the “Program”).

I acknowledge that my participation or the participation of my child is voluntary.

I knowingly and willingly consent to grant consent for my child to participate in the Program, and I along with my child agree to follow all guidelines and recommendations to prevent the spread of COVID19.

I represent that my child has the maturity, understanding, and skill to safely engage in the Program and all other Hockey School activities and to observe all protocols and guidelines for such participation as stated in the COVID-19 Update.

I understand that engaging in the Program can be vigorous, physically demanding, and could result in injury.

I also understand that the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms and may still be highly contagious, and I agree that it is impossible to determine who has it and who does not, given the current limits in virus testing.

I further understand that my child’s participation in the Program involves a risk of being exposed to, contracting, or being around players who have COVID-19.

I will do my part to follow all protocols, respect the directives, and abide by the requirements to participate.

I represent that my child is in good physical condition and does not have any medical, physical, or mental conditions that could result in injury or illness for others.

I expressly assume the risk of my child engaging in the Program and other Hockey School activities and hereby release and hold harmless the Hockey School and all parties involved, including the owners, coaches, other players and families, the staff of “Coast 2 Coast Player Development” and “Coast 2 Coast Hockey Institute”, the staff and owners of Northwoods Ice Center, and local municipalities.

In consideration of being afforded the opportunity to participate in the Program, and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, I, and my minor child, for and on behalf of myself and my heirs, executors, administrators, successors, and assigns, hereby forever release, acquit, and discharge, and agree to indemnify and hold harmless, Coast 2 Coast Player Development and Coast 2 Coast Hockey Institute and its operators, officers, directors, employees, contractors, members, partners, agents, sponsors, vendors, invitees and affiliates from any and all causes of action or actions, suits, losses, liability, damages, claims (including but not limited to, negligence, gross negligence, willful or intentional conduct), personal injury damages, medical or hospital bills, lost wages, property damages, judgments, levies, and executions whether known or unknown, liquidated or unliquidated, fixed or contingent, direct or indirect which the undersigned or his heirs, executors, administrators, successors, and assigns may have, against said Releasees arising out of my or my child’s participation in the Program and any other Hockey School or camp activities.

I understand and agree that the foregoing release is binding on myself and my heirs, executors, administrators, successors, and assigns and that the Hockey School will deny my participation in the Program until this release is executed by myself and/or my child (if applicable) and all terms are hereby accepted.

I understand that, if I am a minor, my parent or legal guardian is required to sign this release.

I understand and agree with each and every matter stated herein, I knowingly and willingly consent to my child’s participation in the Program, and I hereby bind myself and my minor child to the terms of this Waiver by clicking the designated box below, by providing an electronic signature, and by submitting this form.

If any provision of this Waiver is invalid, illegal, or unenforceable in any jurisdiction, such invalidity, illegality, or unenforceability shall not affect any other term or provision of this Agreement.

I have read, understand, and accept the Waiver as outlined above.

I also understand and agree that by submitting this form my email, timestamp, and electronic signature will serve as my original signature.

I further agree that “Coast 2 Coast Hockey Institute” & “Coast 2 Coast Player Development” is authorized to obtain and authorize emergency medical treatment for my child up to and including emergency hospitalization. I agree to be personally responsible for any related medical expenses.
