Terms and Conditions Details
1. MONTHLY FEE: Monthly Fees are non-refundable and must be paid before 1st of each month. Late fee charges maybe applied for late payments.
2. MAKE UP CLASSES: No make up classes are provided for missing classes.
3. WITHDRWAL POLICY: If a student/parent decides to withdraw the student from the program, at least one month prior notice should be given. Otherwise, the monthly fee will be charged.     
4. Services for students who do not follow rules and guidelines maybe terminated without prior warnings
5. If the student needs special attention or have a medical condition that the management needs to be aware of should be disclosed.
6. TRIAL CLASS POLICY: Student shall take a maximum of one Trial Class. If the student/parent decides not to continue after the trial class, no payments are required. If the student/parent decides to continue with the classes, the trial class is considered as the first paid class. 
7. The person who is signing up this form is at least 19 years of age and is the parent or the guardian of the child
