Intermediate Library Parental Consent Form

State Bill 13 requires school districts to provide parents full access to their children's library records and allows parents to create lists of library materials their children cannot check out. If you wish to restrict your child from accessing specific titles or authors, fill out this form and it will be sent to the campus librarian.
