All artists or organizers must complete this form to identify potential safety hazards in their exhibit. This ensures compliance with MIT’s EHS guidelines and allows for necessary safety measures. If your project or artwork includes electrical components, suspended objects, hazardous materials, working from heights, building large structures, etc. additional review may be required. Submit the form at least 2 weeks before the installation to ensure a safe exhibition for all.

Brief and clear description:
Provide the dimensions of your project, including height, width, and depth. Specify whether it will be installed at or above ground level and indicate its approximate total weight.

What materials will be used in your project? Are they treated with fire retardant? Use materials that are non-combustible or can be effectively fire-retardant treated.

Per Massachusetts Fire Prevention Regulations, highly flammable materials are prohibited in exit stairways and corridors. Ensure all materials comply with fire safety guidelines.

If electricity is involved please coordinate with the space manager to submit a work order and reach out to John Ledbury - MIT Senior Electrical Supervisor at jledbury@mit.edu to review your work order.

How will you ensure your safety and that of others while constructing, installing, or setting up your project? Consider potential risks such as overhead hazards, sharp or rusty edges, structural stability, electrical safety, and trip or slip hazards. Your setup should not pose any danger to yourself or others.
