CC+ Intent Contract 2021-2022
This form is the Centerville City School District's student intent form. This form must be filled out for students wishing to participate in College Credit Plus (CC+). This intent form must be completed annually for those students wishing to continue in the CC+ program. Please read the information carefully so that both the student and parents understand the information related to the CC+ program. The College Credit Plus Intent Contracts needs to be submitted electronically, the contract must be time stampeded by April 1st in order to participate in this program. If a student fills out the Intent Contract, they are not compelled to take CC+ courses. If you have any additional questions, please contact our Unit Counselor. My advice is to have this form filled out PRIOR to Spring Break.
