Application: Math 72SL Classroom Practices in Middle School Math, Fall 2017
To be eligible for Math 72SL, you must have completed Math 31A and Math 31B with a C- or better (or equivalent), and be planning to enroll in Math 32A (if you haven't already). It is highly recommended that you have taken Math 71SL (Classroom Practices in Elementary School Math), although not required. Typically, first year students enroll in Math 71SL and second year students enroll into Math 72SL, but this course (Math 72SL) is open to students in their third, fourth, and fifth years as well.

You will be able to enroll through MyUCLA in Math 72SL once provided a PTE number. The seminar is scheduled for Wednesdays from 4:00 - 5:20 p.m. in 6201 Math Sciences Building.

Additionally, you will be expected to spend approximately 2 to 4 hours per week in one of our partner middle schools, Lincoln Middle School, for a total of 24 hours over the quarter. The field experience will be scheduled based on your availability and will vary by student. The internship will begin during the second week of the quarter. All 24 hours of internship to be completed by the end of finals week of Fall Quarter.

Proof of negative TB status is required to participate in the field component of this course. TB tests must be taken within 6 months of starting the internship, and therefore should be dated April 9, 2017 or later.

You should plan to bring a copy of your negative TB test to the first seminar, or you may email a copy to cateach@chem.ucla.edu.

See Cal Teach website for more information about specific requirements for the TB test. Please be aware that appointments at the Ashe Center often fill quickly at the beginning of each quarter (and it may take up to two to three weeks to receive an appointment) so please plan ahead and make your appointment well in advance, if necessary. You can also go to other clinics such as CVS Minute Clinic or WellnessMart MD or your own personal physician (see our web site for more info).

TB Test:
I understand that I cannot attend my assigned school until a copy of my negative TB test is on file at the Cal Teach office at 1039 Young Hall, and that it must be dated within 6 months of starting the internship. I further understand that it may take the Ashe Center up to two to three weeks to schedule an appointment for me, and therefore I should make my appointment as soon as possible once accepted into the course, if necessary. I am aware that I should check with the Cal Teach office for other options should I not be able to schedule a TB test in a timely fashion at the Ashe Center.
