This form is for the guardian of the Kindergarten student to list the people they authorize to meet their kindergarten student at the doors of the bus/van in the afternoons. Please list as many people as needed to meet your child, if there is nobody at the stop to meet the child they will be returned to Wallace for you to pick up.
