If you wish to report something that poses an immediate risk to life or an immediate risk of serious injury during an event YOU MUST FIRST REPORT THIS TO THE EVENT ORGANISATION (e.g. a Ranger, Site Lead, Security, Medics, emergency services - depending upon the event and incident).

Form Introduction
This form is a streamlined single reporting mechanism for Burning Nest and London Decompression.
A member of the Consent Team you have selected will process and progress your report.

By having a single form, it reduces the burden on people making reports having to repeat themselves.

This form is for reporting adverse interpersonal interactions that have resulted in physical and/or psychological harm, or may result in significant risk of this harm, in relation to the UK Burning Man community.

Whilst there is an expectation for burners to deal with conflicts between themselves or use other official channels like the police or a trained meditator, the event Consent Teams does support those who are not able to speak out for whatever reason.
Therefore we accept relevant information that helps us reduce the risk to the UK Burning Man community as a whole.

This form can be used during a burn event and outside of event time, the processing of information depends upon how the submitted information relates to an event and specific event policies.

Breaches of the event code of conduct, terms & conditions or event specific violations not related to consent should be reported to the event organisers.

You can submit anonymous information via this form.

If your report relates to a member of an event Consent Team you must not use this form and instead directly contact the event Consent Lead.
If you report is about the event Consent Lead contact the Site Lead (if during event time), or the Event Lead or Event Directors/Trustees (if outside of event time).

This form should take no more than 20 minutes to complete after the reading Section 1 (depending upon how much information you wish to provide).

Section 4 - Who is your report about (details about the subject)? [3 minutes]
Section 5 - Incident Information (when, where, what) [3-10 minutes]
Section 6 - Your identification (you can remain anonymous) [1 minutes]
Section 7 - Your details (if you wish to be known / contacted) [2 minutes]
Definitions
Subject - the person of concern or the person responsible for the incident.
Aggrieved - the victim/survivor of the incident.
Reporting Party - the person making this report (may or may not be the same as the aggrieved).
Data Processing Consent
All submitted forms will be reviewed and processed by the event Consent Team(s). They will be progressed and escalated as required.

By submitting this form, you consent to the event Consent Teams to use any personal information supplied for the legitimate interest of the safety of the events, including contacting you if you have provided your details.
