Client Periodical Progress Assessment
The Client Periodical Progress Assessment measures 25 domains of need relative to the client's self-sufficiency and progress in each area of need. Use this assessment to track progress in applicable domains of need at intermittent follow-up periods (i.e. monthly, quarterly, etc.).

When administering this assessment, keep in mind that all situations may not be represented but please choose the answer that best matches the client's current status in each area. Also consider that for a client to be in an Empowered status for any of the following items they should demonstrate that they have no identified area for improvement in need in the domain.

Additional note: If the answer to any question is "does not apply" this will not have any positive or negative bearing on the results.
