Old Form: Use Client Portal for Records Requests

This legacy form is no longer utilized for client requests for Protected Health Information (PHI) or records.

To submit a new records request, please utilize the new form located within the SonderMind Client Portal:

Note: At this time, this must be done on the Desktop version of the portal. Access via the mobile app is forthcoming.

1. Log in to your SonderMind Client Portal.
2. Navigate to the 'Account' tab.
3. Scroll down to the 'Records Request' section and complete the request form provided there.

🛡️ Policy on Third-Party Requests

In an effort to protect client privacy and confidentiality, we only accept requests for records directly from the client, unless legally (e.g., court order) or contractually (e.g., requests from payors) obligated.

If you are a third party (e.g., a law office, doctor's office, etc.), please have the SonderMind client fill out the new form.

The client is able to designate a third party to send the records to within the new portal request form.
