The Summer Program (SSP) 2025              Pre-Enrollment Form

The 2025 Summer Program will operate from 7:00 AM to 6:00 PM at the following locations:

   •    Helen Tyson Middle School (Springdale)

   •    Folsom Elementary School (Farmington)

   •    The ASP Early Childhood Academy (Lowell) *Limited number of Pre-K enrolled at this location

The dates of operation will be: June 2 – August 1, 2025. Dates of operation are subject to change based on the school calendar.

Age requirements: Children must be at least 5 years old by June 2, 2025, and cannot turn 14 years old at any point from June 2 - August 1, 2025.

*The application process consists of timestamp, grade availability, currently enrolled, past behaviors, past billing history, parent/guardian behaviors, etc.

$100 registration fee per child
1 child: $190/week tuition
2 children: $370/week tuition
3 children: $530/week tuition
4 children: $630/week tuition
I understand that the 2025 Summer Program is only accepting full time enrollment, and that I will be responsible for the full rate of tuition from June 2 - August 1, 2025, regardless of attendance. *
Please list any special schedule requests. If full-time enrollment fills up and availability remains, special schedules will be considered.  *
