CSD Meals Vegetarian & Special Diet Form

Please fill out this form to secure your child's vegetarian meals 𝗢𝗥 special diet meals. Parents requesting for special diets are required to have email communication with our Nutrition Supervisor Sarah Catherine Black (sblack@csdecatur.net) after filling out this form.

Please complete a separate form for each child.

𝗪𝗵𝘆 𝗮𝗿𝗲 𝘄𝗲 𝗮𝘀𝗸𝗶𝗻𝗴 𝘃𝗲𝗴𝗲𝘁𝗮𝗿𝗶𝗮𝗻 𝗮𝗻𝗱 𝘀𝗽𝗲𝗰𝗶𝗮𝗹 𝗱𝗶𝗲𝘁𝘀 𝘁𝗼 𝗳𝗶𝗹𝗹 𝗼𝘂𝘁 𝗮 𝗴𝗼𝗼𝗴𝗹𝗲 𝗳𝗼𝗿𝗺? We are preparing to-go meals for ALL our students who are learning in-person in our CSD buildings or even learning virtually at home. Our department has been provided with the total number of students at each school site. However not all of these students will want vegetarian meals or they won't have a special dietary need. The purpose of this form is to help us predict the demand for vegetarian and/or special diet meals at all our school sites and to know where to send meals for our virtual students.

𝗪𝗵𝗮𝘁 𝗶𝗳 𝗺𝘆 𝘀𝘁𝘂𝗱𝗲𝗻𝘁 𝗶𝘀 𝘀𝘁𝗶𝗹𝗹 𝗹𝗲𝗮𝗿𝗻𝗶𝗻𝗴 𝘃𝗶𝗿𝘁𝘂𝗮𝗹𝗹𝘆? Please fill out this form! We will have meals available for pick-up at 4 different TakeOutSpot locations depending on their current grade level. (Please select Fifth Avenue or Talley Street if you have a K-2 student.) 𝗦𝘁𝗮𝗿𝘁𝗶𝗻𝗴 𝗠𝗼𝗻𝗱𝗮𝘆 𝟯/𝟴 there will be TakeOutSpots at Fifth Avenue and Talley Street for 𝗱𝗮𝗶𝗹𝘆 𝗽𝗶𝗰𝗸-𝘂𝗽 from 11:40 AM - 12:15 PM. Please note that our TakeOutSpots at Renfroe Middle School and Decatur High School will be open for 𝗠𝗪𝗙 𝗽𝗶𝗰𝗸-𝘂𝗽. The time frame at Renfroe Middle School is 𝗠𝗪𝗙 from 12:15 PM - 1:00 PM and the time frame for Decatur High School is 𝗠𝗪𝗙 from 12:45 PM - 1:45 PM. We still need to be able to predict the number of vegetarian meals or special diets that will be picked up at these sites. By filling out this form, you will be helping us better forecast our vegetarian meal counts.

𝗪𝗵𝗲𝗿𝗲 𝘄𝗶𝗹𝗹 𝗺𝘆 𝗶𝗻-𝗽𝗲𝗿𝘀𝗼𝗻 𝗰𝗵𝗶𝗹𝗱 𝗽𝗶𝗰𝗸 𝘂𝗽 𝘁𝗵𝗲𝗶𝗿 𝗺𝗲𝗮𝗹𝘀 𝗮𝘁 𝘁𝗵𝗲𝗶𝗿 𝘀𝗰𝗵𝗼𝗼𝗹? We are going to send all vegetarian meals and all special diet meals to the front office of all CSD buildings. Please remind your child that they must stop by the front office as they exit the building to receive their food. As we refine these processes, this pick-up location MAY change depending on the school site.

𝗪𝗵𝗮𝘁 𝗶𝘀 𝗮 𝘀𝗽𝗲𝗰𝗶𝗮𝗹 𝗱𝗶𝗲𝘁? A special dietary need is one that requires medical documentation by filling out the form "Medical Statement for Accommodating Disabilities" on our Special Diets Webpage. Please email sblack@csdecatur.net with the filled out form to begin the necessary steps for dietary accommodations. Note that it may take some time to ensure that all of a specific child's dietary needs can be met by our department. We will do our absolute best to ensure that we can meet your child's dietary needs while also considering the needs of our department.

𝗥𝗲𝗺𝗶𝗻𝗱𝗲𝗿: ALL meals are FREE of charge until the end of the school year. By taking a daily meal from us, you are supporting our nutrition program and helping our department function as it normally would.

The CSD Nutrition department is operating under a meal program called "Seamless Summer Option," which is supported by the USDA. As we transition to this new program all students will be provided BOTH breakfast and lunch meals. We will not be allowing students to opt out breakfast. We encourage you to try our breakfast as it's an important part of a healthy diet! *
