National PTA Giveaway Entry Form: Request for Free Lysol® Disinfecting Products
Please supply the information below for an opportunity to receive free disinfecting wipes and spray from Lysol®, a promotion in conjunction with Teacher Appreciation Week.
Opportunity:
National PTA will randomly draw 100 PTAs* from the entries received.
What Your PTA Will Receive:
Each chosen PTA site will be mailed 4 cases of Lysol Disinfecting Wipes (6 units per case of 80 count flatpack wipes) and 2 cases of Lysol Disinfectant Spray (12 units per case of 12.5oz cans).
Deadline & Notification:
Entries will be accepted until May 7, 2021 at 7 PM EST. National PTA will notify recipients by email by May 28. Lysol® then will be provided the addressee information from this form to ship the products.
Delivery:
Lysol anticipates that all products will be delivered to coincide with the start of the 2021-2022 school year.
Eligibility & Rules (Other rules and restrictions may apply):
* PTAs must be in good standing with their State PTA.
* Unfortunately, this opportunity cannot be extended outside of the contiguous United States and is therefore not available to PTAs in AK, HI, PR, USVI, or Europe.
