LCUP Registrar Online Request of Credentials 2025
For onsite daily transactions, kindly adhere to the following:

1. The Health Services Unit has the authority to approve or deny entry to the campus-based on the standard protocol observed within the university.
2. Persons exhibiting flu-like symptoms  are not allowed to enter the campus or should wear face mask.
3. Temperature screening is required  before entering the campus premises.
4. Bring your own ballpen and alcohol.
5. You will receive the first email confirmation within 48 hours of completing the request form.
6. Students with accounts payable or balances must coordinate with our FINANCE OFFICE prior to the processing of Document Requests.
7. Release of requested documents is based on the schedule sent to your registered email by the Office of the University Registrar. Make sure that the email address you used to make the request is still active.
8. For authorized representative/s: Kindly bring authorization letter of the student and valid ID of both representative and student requesting the document.
9. All onsite transactions (releasing of documents) are strictly by APPOINTMENT only.
10. Check your SPAM messages regularly.
