This form is to be filled out by club directors or admins only. This form does not register an individual for a particular club's tryouts - it is used by clubs to give their tryout information to the CEVA office.
This form may only be completed by active clubs. Use this form to request sanctioning for your tryouts in the 2025-2026 season from CEVA. If you need to change any of your information after submission, please e-mail
region@cevaregion.org. The information you provide will be posted, if requested, on the CEVA website once your club is fully activated within the region. If any information changes after submission or posting, it is the responsibility of the club to inform the CEVA office of those changes.
Teams and individuals who wish to participate in CEVA's Power League, Regionals, or other sanctioned programming must adhere to CEVA's tryout policies.
