MPS Commitment Form (Due March 5, 2021)
The MPS Commitment Form provides details for Remote and Onsite learning options. Parents will use this form to select the option for learning beginning the fourth nine weeks. Complete a form for each child attending an MPS school.
NOTE: When parents decide how a student will return (on school site or work remotely), the student must remain in the selected option (on-site or remote) for the nine week period. Special consideration may be granted for emergency/critical situations. This will be determined on a case-by-case basis.
If onsite learning is selected, any day the student does not report to campus will be considered an absence and an excuse will be required. This onsite learning option may have to be adjusted based on the pandemic.

1. All MPS Chromebooks must be turned in March 15 - 18, 2021 to prepare for mandated standardized assessments. All
students will be using learning packets during this time. Schools will schedule the drop-off time.
2. Students will be scheduled to come onsite for required standardized assessments beginning March 30th. Parents will be
notified regarding the schedule by school administration. Transportation will be provided.
3. All students will work remotely using learning packets March 29-April 2, 2021.
