* The person listed below will also receive documentation to provide to a tax advisor to determine if it is tax-deductible.
* If you are submitting this form for a basket that contains items from multiple donors, we ask that you fill out this form for each separate item (so that staff can provide tax documentation to each donor for each item).
* Please be ready to upload a photo of your donation or a logo/image for your organization.
