Thank you for nominating a facility to participate in our Healthy Habitats Gardening Program!

Facilities to consider: Skilled nursing, rehabilitation, memory care, assisted living and more!

Selected facilities will receive all materials for the installation and maintenance of a Pollinator Habitat Garden, alongside educational programming and Butterfly Pavilion support in the installation process! Opportunities for additional workshops will also be provided throughout the year - including native seed growing, butterfly rearing, continued garden care, and more!

We will reach out to each nominated facility as is possible.
