Northgate High School CARE TEAM Referral Form 25-26
Please complete this form to refer a student to the Northgate High School CARE TEAM. The CARE TEAM is made up of Northgate's school counselors, crisis counselors, school psychologist, school nurse, and a Vice Principal.

Students should be reported to the CARE TEAM anytime you are concerned about their academic or personal well being.

Once this form has been submitted, the CARE TEAM will begin working with this student, and report back to you an update when appropriate.
