Survey of People Using Beth Israel Medical Center

The Mount Sinai Health System wants to close Beth Israel Medical Center, located at 1st Avenue and 16th Street. The Community Coalition to Save Beth Israel has put together this survey is to find out how you or your loved ones use Beth Israel Medical Center and how you would be affected if the hospital were closed. We especially need to hear from people in groups designated by the state as often "medically-underserved," such as people who have low incomes or are LGBTQ+, women, Black, Latino/a/x, immigrants or people with disabilities. So please take care to answer the questions at the end. Also, please consider giving us your email so we can follow up to get additional information. Your answers  will be kept confidential unless you give us express permission in the future to share your story. Your answers will help us fight to save Beth Israel!
