WCM - Craven County Schools Parental Agreement Form
All parents, guardians, and care providers are required to complete this form for your child’s school. This form will notify the school if you give permission for your child to participate fully in school activities.
You may give your permission to all the activities or choose to select the ones with which you agree. Please note that if you decide to deny permission in a program, your child will not be able to participate or be recognized in some activities. The information for each activity is located in the Student Policies and Regulations Book.
