Fill in and submit the following form for each drill that you are NOT able to attend. ​

Please include the date of the drill along with the specific reason for absence (family travel, mandatory school activity, sickness, etc).

In order for the absence to be excused, you must submit this form no less than 48 hours in advance.

Remember that you must attend 75% of drills each quarter and failure to meet that standard can be grounds for dismissal.

Your submission of this form does not guarantee that your absence will be excused.

Each absence will be evaluated on a case by case basis.
