It is extremely important that we know your plans for your children for the first trimester so we can plan accordingly and be responsible for their safety.

We know that occasionally you will need to deviate from the regular schedule. When this need arises, please send a note to the office in the morning before the start of school indicating the change.

Please complete this form for each of your children in grades 3 through 8 by August 21.
